Each form takes a few minutes on your phone or computer. You sign on the screen with your finger or mouse. When you have finished your forms, press Finish and you and the dive center receive all of them in one email as a single PDF (if you forget, they are sent automatically 30 minutes after your last form). Forms filled on one device are always sent together in one email (one PDF per diver) – when the diver presses Finish, or 30 minutes after the last form. Staff checking in a boat or a family can also start a named group and send it themselves.

Which forms do I need? Every diver fills in forms 1 to 4 and 6. Form 5 is for certified divers who take a course with us (Advanced, Rescue, Divemaster, specialties). Discover Scuba Diving guests need forms 1, 2, 3 and 6.
Under 18? A parent or guardian signs each form too – either on the spot in the second signature box, or we email them a secure link to sign on their own phone or computer.
Answered YES on the medical? Download the Diver Medical form (PDF), have your doctor complete page 3 (Medical Examiner's Evaluation Form) and upload it in the medical questionnaire; it cannot be submitted without it.
