Fill inWhich forms do I need? Every diver fills in forms 1 to 4 and 6. Form 5 is for certified divers who take a course with us (Advanced, Rescue, Divemaster, specialties). Discover Scuba Diving guests need forms 1, 2, 3 and 6.
Under 18? A parent or guardian signs in the second signature box of each form.
Answered YES on the medical? Bring the PADI Diver Medical Physician's Evaluation Form signed by your doctor, or ask us for a copy.
